Pharmacist – 22.5 hours/week – Permanent

Family Health Teams (FHT) use a patient-centered approach to primary care that brings together doctors, mental health clinicians, nurse practitioners, registered dietitians, registered nurses, registered practical nurses and pharmacists to coordinate the highest possible quality of care for patients- when they need it – as close to home as possible. FHTs, in collaboration with Family Health Organizations, are a key component of Ontario’s healthcare plan to reduce wait times, improve access to primary care and keep Ontarians healthy.

Overview

Primary Care Clinical Pharmacists are an active member of a multidisciplinary team who are the medication management experts of the health care team.  They practice patient-centered and evidence-based care to achieve optimal drug therapy outcomes for rostered patients, while collaborating with the multi-disciplinary team of primary health care professionals.

Responsibilities:

  • In general, provide direct patient care in partnership with patients and other health care providers.  Specific activities will depend on the specific care that the pharmacist is providing and could include:
  • Anti-Coagulation Clinic Pharmacist – determining INR and monitoring warfarin dosing as per a Medical Directive
  • Diabetes Care Pharmacist – monitoring specific parameters and adjusting treatment as per a Medical Directive
  • Smoking Cessation Pharmacist – helping patients achieve their goals through medications and counselling support
  • Medication management – identifying drug related problems through medication reviews and patient interviews and following through to resolution
  • Drug information – acting as a resource for physicians, nurse practitioners, and other allied health professionals in providing evidence-based, timely and unbiased drug information.
  • Health education – working individually and in groups to educate patients and caregivers to help them better manage their health
  • Participate in program development within The Peterborough Family Health Team focusing on chronic disease management initiatives.
  • De-prescribing – working with patients and their physician and/or nurse practitioner to optimize and rationalize their current medication use and attempt to reduce it with a particular emphasis on the elderly, in palliative care, in insomnia, and in opioid addiction.

 Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.

Hours of Work:

Monday to Friday – Hours of service are pre-established per employment contract and may require some evenings and weekends. Any deviation from the established schedule shall not negatively impact upon the provision of service to the patient population.

 Qualifications

  • Bachelor of Science in Pharmacy from a recognized university
  • Registration with the Ontario College of Pharmacists (Part A)
  • Two years’ experience preferred, or community/hospital residency and/or Pharm D an asset: involvement in Primary Care preferred
  • Additional training or credentialing in pharmacotherapy (eg. Certified Diabetes Educator, PHARM-MOAT – Management of Oral Anticoagulant Therapy Course from U of Waterloo, CAMH TEACH certification) will be necessary depending on the specific pharmacist’s role.
  • Additional training or credentialing in pharmacotherapy (eg. Certified Geriatric Pharmacist, Certified Asthma Educator, etc) considered an asset.
  • Ability to work both independently and effectively in an interdisciplinary team environment
  • Experience in providing patient-centred care and evidence-based unbiased drug information
  • Proof of professional liability insurance
