Having a family doctor is important for continued care. Family doctors provide care to people of all ages, in each step of their life.

Currently, the only way to “sign-up” for a family doctor is through the provincial Health Care Connect program.

Peterborough Family Health Team is unable to connect you with a family doctor.

If you require non-urgent care, you can visit the PFHT Health Clinic.

You can register with the program by visiting www.ontario.ca/healthcareconnect or by calling 1-800-445-1822 from Monday to Friday from 9 am – 5 pm

When you register you will be asked for your:

  • name
  • health card number
  • basic health information

Peterborough Country Assessment Form

Those living within Peterborough County (excluding the City of Peterborough) who do not have a family doctor are asked to complete a Health Care Assessment form created by the Health Care Advancement Coordinator. This form will assist in collecting valuable data for each municipality. Click the links below for details:
