Registered Dietitian – 37.50 hours bi-weekly – Permanent

Overview
The Registered Dietitian is an active member of a multidisciplinary team who is responsible for the delivery of patient-centered clinical nutritional counselling services. The services of the Registered Dietitian will serve to enhance the quality of primary health care services in the area. This is a .50 permanent part-time (37.50 hours bi-weekly), position working at the Morton Medical Centre in Lakefield.
Responsibilities
- Identify nutrition problems
- Assess the nutritional status of patients
- Establish goal oriented treatment interventions
- Provide family/patient education and provide support to address nutritional needs
- Develop care plans and monitor the effectiveness of dietary changes
- Provides individual treatment and group classes as needed
- Specialize in diabetes, cardiovascular diseases, gastrointestinal conditions, and malnutrition
- Provide support to adults and seniors, including those with special medical needs
- Act as a coach/mentor/consultant to patients
- Act as a resource and participate in Family Health Team educational programs
- Regular attendance to all FHT meetings is mandatory
Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.
Hours of Work:
Monday to Friday
Some occasional evening work may be required.
Qualifications
- Registered Dietitian with the College of Dietitians of Ontario
- Two years’ experience in a primary health care setting preferred
- Current Diabetes Educator certification an asset
- Excellent assessment, planning and organizational skills
- Knowledge of the principles of adult learning
- Demonstrated ability to facilitate both individual and group educational sessions using the principles of adult learning
- Experience with Telus PS Suites an asset

- Demonstrated proficiency related to behaviour change counselling

- Capacity to adapt quickly to a fast paced, dynamic work environment
- High degree of accuracy and attention to detail
- High degree of critical thinking skills
- Proven participatory work style; ability to work independently and with a team
- Basic computer skills
- Demonstrated commitment to continued education
- Demonstrated good attendance with ability to maintain this standard
